Default ACSLS Missing volume

Has anyone encountered a 'missing' volume in their library?

I have an L5500 and ACSLS 7.1. Somehow a tape ended up outside the
library and is now listed as 'missing' when I query the volume in
cmd_proc.

When I try to inject the tape, it gets scanned (cap set to automatic),
and then the cap gets changed from enter to eject, and the only message
in the acsls event log is to remove the volume from the CAP.

Any suggestions on how I can get this tape back in the library?
