Default nVidia Quadro NVS 440 Wavy and Blinking One Monitor

I have a Quadro NVS 440 set up with four HP LP 2035 LCD monitors, each
running 1600x1200 and the card is set to 32-bit.

After being stable for a few years, starting today, just monitor #3 of four
monitors at the left part of the screen has started to blink and become
unstable and wavy. The effect is a bit like some kind of interlacing
where the two passes do not register to the correct pixel locations, so the
text at each position appears to constantly shift.

What would cause this problem and how can I reset it? Would this problem
indicate the monitor is going bad?
