Wintel HBA - Logical to Physical

Default Wintel HBA - Logical to Physical

Seem's like a straightforward request "How to map the logical -
Adapter x, to the physical - Slot y?" but it seems extremely difficult
on wintel machines. There are utilities like lputilnt and
hbaanywhere, etc for the emulex cards but even these don't show this
information. I have tried fcinfo, this shows \\.scsi3 but then how to
you map this to the cards listed in device manager?

Does anyone have a utility or advice on how to achieve this?
