Default dead USB ports too frequent for my liking

I've lost zero USB ports here.

One of my secrets, is I never wire up USB front
panel connectors. I use the rear connectors
exclusively. That's why they don't fail.

I've had rear USB ports fail on two motherboards, in both cases
because the power supply's +5Vstandby circuit suddenly put out
way more than +5V, +13V in one case. I also had a port on a
PCI USB 2.0 card based on an NEC USB chip fail because the
LM3526 power switch chip failed and kept the port from getting
any +5V power.
