Default Fabric OS - switch won't initialize?

We have a EMC DS-16B switch that boots, however never gets
to be in an online and ready state. At the prompt most
commands reveal "Switch not ready", and subsequent
logins produce the message "WARNING: switch still
initializing - please wait".

The problem is it *never* finishes initializing. Forcing
it online using the front panel shows an online status in
switchshow, however this makes no difference as the switch
remains "not ready".

The boot sequence looks like this:

-TELNET-I-ESCAPE, Escape character is ^]

Fabric OS (tm) Release v2.4_test

login: passed.

Running Port Register Test .... passed.

Running Central Memory Test ... passed.

Running CMI Test .............. passed.

Running CAM Test .............. passed.

Running Port Loopback Test .... passed.
Fail to get secure mode from flash.

Fail to restore security mode.

errshow simply displays the above status change message, and
switchstatusshow the following:

The overall switch status is Marginal/Warning
Contributing factors:
* Switch Offline triggered the Marginal/Warning status

Many thanks for any pointers in troubleshooting this problem.
